Charity Tooth Whitening March 1, 2001 to June 30, 2001
To raise money for children's health related charities the dentists provide tooth whitening for patients at a
reduced fee and all the money goes to charity. The patients donate a minimum of $275 to Smiles For Life,
the doctors non-profit foundation, and they do the tooth whitening for you. The whitening materials are
donated by Discus Dental and the doctors pay all the expenses so that every penny goes to the children.
You even make your check out to the foundation, Smiles For Life, and they mail your check or credit card
receipt to the foundation headquarters in Salt Lake City, Utah.
This program was started in 1998 and has raised nearly $9 million in the three years. Dental Care of
Stamford has been the number one office in the country every year and has raised nearly
$325,000.
In year 2000 Smiles For Life joined with Garth Brooks non-profit foundation called Touch'em All to expand
our ability to raise money and help administer the distribution of the collected funds.
Please help us make 2001 a record breaking year for Smiles For Life. It is a win-win program because you get a whiter brighter smile at a great price that may be tax
deductible, the dentists win because we get to contribute and make a difference, and of course the kids
win because they get the medical care they need to get well. Please come and participate and bring all you friends. It is a great deal.

All the money collected is donated to children's
hospitals such as St. Jude.

All Foundation to raise money together.

New York Mets baseball team manager,
Bobby Valentine, participated in the fund raiser with Dr. Simon this
year.
Dr. Simon offers his personal thanks and
appreciation to the over 1500 people who whitened their teeth and donated
to Smiles for Life.

Smiles for Life
Charity Fund Raiser Benefiting St. Jude Children's Hospital
Smiles for Life raised $ 1.7 million
dollars in 1998 and raised 2.8 million dollars in 1999. All
of the money went to St Jude Children's Hospital.
In 2000 Smiles For Life joined with Garth Brooks Touch'em
All Foundation and we donated 3.8 million to Touch'em All. Touch'em All additionally raised
another 2 million from other sources for a total of nearly 6 million dollars. Since the dentists
pay all of the expenses for Smiles For Life, and Garth Brooks pays all the expenses for Touch'em All, all the money raised goes to 40 institutions that are all children related health care
organizations.
The plan this year, 2001, is for Smiles For Life to raise 5 million dollars by tooth whitening.
